Patient Registration

Patient Registration with the Practice

Any new patient(s) wishing to register with the Practice will be required to visit the surgery to complete a new patient registration form and a new patient health questionnaire form.  Both forms need to be signed by the individual patient wishing to register (children's registration forms can be signed on their behalf by a parent or legal guardian).

Once registered it can take a number of weeks before we receive your medical records therefore it is important that the medical questionnaire is fully completed.

Due to the length of time it can take to receive your medical records, it is important that you register with the practice as soon as possible and not leave it until you are ill or need our services.

All medical practices work within specific catchment areas therefore if you have any doubts whether you are in our catchment area please contact the practice.
